What You Need to Know Before Your Appointment

  1. Be ready to inform us in detail any vision problems or symptoms you are having (difficulty seeing at night, flashes of light, spontaneous double vision, blurred or loss of vision, etc).
  2. Provide a list of all medication you are taking (prescription and non-prescription medication).
  3. Be ready to discuss any history of eye problems, eye injuries or eye surgeries you or your family has had (glaucoma, cataracts, etc. as well as dates and where treated).
  4. Bring your current eyeglasses and/or contact lenses.
  5. Bring your insurance information for vision care.
  6. List out any questions you may have for the doctor.
